The chapel was built in 1905 and is located in Mały Cisiec in Gmina Węgierska Górka. It was sponsored by Michał nad Anna Strzałka (the Porębscy).
The view of the small chapel by the road to honour Madoona with the Child allows to see the Madonna small chapel in the half of the mountain.
In front of the small chapel there is a low-relief that shows a kneeling figure of St. Dominic. On the left side, there is a low relief of St. Michael. On the right side, there is a figure of St. Anne with her little daughter Mary.
The figures of children on the chapel show the profound faith of the people from Cisiec both now and in the past. The chapel also harmonizes with the scenic surrounding perfectly.
The picturesque small chapel dedicated to Madonna with angels and St. Andrew Bobola is located on the meadow in Cisiec.
